Apex provides wellness, aesthetic, and personal care services including but not limited to:

• UV tanning
• Red light therapy
• Teeth whitening
• Skincare and aesthetic treatments
• Wellness services
• Memberships and packages
• Retail product sales

Services may change at any time without notice.

All services are intended for individuals 18 years of age or older. Valid identification may be required. Minors are not permitted unless explicitly allowed by law and accompanied by required consent (if applicable).

Apex SunTan & Wellness Club does not provide medical advice or medical treatment.

You acknowledge and agree that:

• Certain services involve inherent risks, including but not limited to skin sensitivity, irritation, burns, allergic reactions, dehydration, or temporary discomfort
• Results vary by individual and are not guaranteed
• You are responsible for disclosing any medical conditions, medications, pregnancy, or sensitivities prior to services

By using our services, you voluntarily assume all risks associated with participation.

Information provided by Apex, whether in person, online, or through marketing materials, is for educational and wellness purposes only and is not a substitute for professional medical advice.

Consult a licensed medical provider before beginning any wellness or aesthetic service if you have health concerns.

• Appointments must be cancelled or rescheduled within the timeframe stated at booking
• Late cancellations or no-shows may result in fees or forfeiture of prepaid services
• Apex reserves the right to refuse service for repeated no-shows or policy violations

• Payment is due at the time of service unless otherwise stated
• Memberships and packages are non-transferable
• All sales are final unless otherwise stated in writing
• Unused services may expire according to membership or package terms

All services, memberships, and retail sales are final. No refunds are provided for:

• Services rendered
• Missed appointments
• Dissatisfaction with results

Exceptions may be made at Apex’s sole discretion.

We reserve the right to refuse or discontinue service to anyone for:

• Unsafe behavior
• Harassment or inappropriate conduct
• Failure to follow staff instructions
• Misrepresentation of health information

All website content, logos, images, branding, text, and materials are the property of Apex SunTan & Wellness Club and may not be copied, reproduced, or used without written permission.

Apex may use third-party systems for booking, payments, marketing, or communications. We are not responsible for the policies, outages, or practices of third-party providers.

To the fullest extent permitted by Oregon law:

• Apex SunTan & Wellness Club shall not be liable for indirect, incidental, or consequential damages
• Our total liability shall not exceed the amount paid for the service in question

You agree to indemnify and hold harmless Apex SunTan & Wellness Club, its owners, employees, contractors, and affiliates from any claims, damages, or expenses arising from your use of our services or violation of these Terms.

These Terms are governed by and construed in accordance with the laws of the State of Oregon. Any disputes shall be resolved exclusively in Oregon courts.
