1. CANCELLATION POLICY:
This document outlines the cancellation policy for our tanning membership program, specifically regarding Electronic Funds Transfer (EFT) agreements. The policy is designed to ensure that customers fulfill their commitment before being allowed to cancel their membership.
Membership Commitment:
All members who enroll in the tanning membership program agree to a minimum commitment period of three months. This commitment is essential for maintaining the sustainability of our services and ensuring that we can provide quality tanning experiences to all members.
Cancellation Terms:
Minimum Commitment Period: Members are required to maintain their membership for a minimum of three (3) months from the date of enrollment.
Cancellation Eligibility:
• Members may not cancel their membership until they have completed the full three-month term.
• Cancellations requested before the end of this term will not be processed or accepted.
Post-Term Cancellation:
• After fulfilling the three-month commitment, members may submit a cancellation request.
• Cancellations must be submitted in person through our designated cancellation form at least 30 days prior to the next billing cycle.
Billing Cycle:
• Members will continue to be billed monthly until a valid cancellation request is received and processed after the initial three-month period.
No Refunds:
• There are no refunds for any payments made during the commitment period, regardless of usage.
2. SIGNATURE:
By signing below I certify that I have read, understood, and agree to the terms of this Agreement in its entirety. I hereby release Apex Teeth Whitening LLC & Apex SunTan And Wellness Club LLC, the owners, operators, and manufacturers from any damage or harm I might incur from use of the facilities or products. No oral representation or promises have been made to me outside of what is stated on this legally binding agreement.
